I have identified the problem in my case.
The problem is the file extension and the following id.
For example:
www.mysite.com/ANormalMKVwithOddExtension.php will not work.
www.mysite.com/test.mkv?id=1 won't work either.
However both of these cases work if the file is an AVI, only MKV exhibits this problem.
